_*\* MUST be combination CMV with minimum 40 foot trailer*_ _*\* Will count Local, Regional or OTR experience*_ _*\* Oil Field, school or driving on private roads does not count towards experience*_ _*\* Military experience is verified at 50% - If a driver has a year of military service with an 88M or 14T, they count that as 6 months*_ _*\* Must have a current valid Class A CDL, not licensed in a monopolistic state (ND, OH, WA, WY, Puerto Rico, and U.

Virgin Islands)*_ *Moving Violations (max in 3 year period)* * No more than 3 in the past 3 years in any vehicle *Policy on License suspensions* * No multiple license suspensions, downgrades, withdrawals in the last 3 years.
*DOT Recordable Accidents* * If only 6 months of experience, no accidents.
* No preventable fatality within the last 3 years * No rear-end over 30 mph, rollover or head on collision accidents within the past 3 years _*\* All DOT Recordable Accidents in the past 3 years require review - Please try and get accident report*_ *Incidents (max in 3 year period)* * No more than 4 incidents in the last 3 years.
* If only 6 months of experience, no preventable incidents or accidents.
*Ticket / Accident / Incident Combo* * Case by Case *Maximum Major Moving Violation (in the last 3 years)* None of the following; * Careless or reckless driving in previous 3 years * Cell phone violations in the previous 3 years * Improper or erratic lane change in the previous 3 years * Following too close in the previous 3 years * Violating State or local law relating to motor vehicle traffic control (other than a parking violation) arising in connection with a fatal accident * Driving a CMV without a CDL or CLP * Driving a CMV without the proper class of CLP or CDL and/or endorsements for the specific vehicle group being operated or for the passengers or type of cargo being transported * Violations concerning railroad crossings while driving a CMV \* No speeding 15 mph or more over posted speed limit in the last year in any vehicle.

Number of Jobs- in the last 3 years* * No more than 4 in the past 3 years * No more than 2 jobs within the last year *Policy Against Unemployment- even if it is accounted for* * Will review *Terminated Applicants* * None in the last 12 months for safety reasons * Will review case by case *Criminal Convictions - How old must they be? * *Based off the conviction date? What are the "silver bullets"?* * Current probations require a letter clearing driver for interstate commerce * No felonies in the last year or in a CMV in a lifetime * No multiple felonies related to drug trafficking, human trafficking, or smuggling in the last 3 years * No sexual offenses or offenses involving harm to a minor.
* Multiple Misdemeanors, Bond Forfeitures, Fine Payment Plans, Probations, and Multiple Deferrals in the last 5 years require approval by supervisor.
* No multiple prior misdemeanors and felonies in a lifetime _*\* Go by conviction date*_ *DUI/DWI- What is the max number and what * *time frames do they need to fall outside of?* * None in the past 5 years * No drug or alcohol and motor vehicle violations in any vehicle in the last 3 years *\* *_*Go by conviction date*_ _*\* No Positive Drug/Alcohol test in the past 5 years.
Must have SAP Program documentation & must be approved by safety*_ *Is Haz-Mat Required? Is there a grace period for* *driver to obtain it?* * No *DOT Physical Requirements* * Driver must have a valid DOT Physical before going to orientation w/ at least 3 months remaining on current medical card * MVT MUST have a copy of the Long Form Physical, Medical Card and CDL-A *Drug Screening* * Urine *Other Endorsements Required* * None *Other Automatic DQ's.
* * No leaving the scene of an accident in any vehicle ever * No Sexual Charges * No Domestic Abuse Charges * No Child Abuse Charges * No felony convictions while operating a CMV * No Positive Drug/Alcohol in the past 5 years * Truck abandonment or unauthorized location without notice at the previous carrier or more than 1 in the period being reviewed * 12 months in the last 3 years of OTR/Regional experience with Tractor-trailer with a trailer of 40’ of more.
